    Population control—
    the really really hard way.

    “Fire”
    That’s an additive, like “burn with fire”

    “Guts”
    That’s sort of problematic, because “their guts are missing”

    And then there’s “Gore”
    Gore, spread all over.
    “Fire, Guts, and Gore”
    Get you pretty far.

    I’ve seen surgeons walk in looking like that before. I suspect he has the whole scene in his head arranged like Misery, but for some reason, I suspect she was a pretty solid doctor who is flaming mad at him in part for his ingratitude (and philandering).
    I can totally see her experimenting on his conquests, though.

      I agree that she was a solid, if Revolutionary Doctor, like a cross between Ignaz Shimlwize and Frankenstein, both of whom where obsessive and persued offenders with violent zeal. Shimlwize died in prison for attacking a proponent of the debunked “perpeperl” theory, all of whom were no different from mass murderers to him. I am sure he would have been suicidal had he felt responsible for a lot of preventable deaths, which I think Vane feels. I have never felt that she hates him for being ungrateful so much as for mistakingly expressing his gratitude in a way she finds utterly intolorable. If the Reverend COULD send her to hell, vane would be ironically grateful, but I grow more convinced with each installment that the only way to kill an elemental wraith is to slay its caster.

    Curioser and curioser… So, Vane’s operation on Hunter was apparently not entirely altruistic and could have required the sacriifice of numerous “patients”. Then Hunter brings her back from the dead and she chases him all over the frontier… All love is selfish, I guess, but it remains unclear who the true villlain is, or if it’s only one of them who’s a villain.
